Name change on passport

Hi

I am traveling to Mauritius in a couple of days and I wanted to make a change of name on my passport as my maiden name is still on it. Do I need to start a fresh with a new application? If yes can I download the passport application and get a signature from the Police Officers here. Any help would be greatly appreciated.

You will need to fill in an endorsement form which is available for download.

You also need to supply evidence for the change, in your case your marriage certificate and a fee of Rs100.
