On the Mauritius Passport application form, there's no mention of the info you're asking about. The only place where contact details is requested is under the section for Particulars of person in whose name passport is to be issued and they only ask for a permanent address.

Are you filling out a different passport form (for another country) or you obtained your form somewhere else?

its the new form application and I did get it fill by a policeman
